CT WiZ: Access My Immunization Record

Get your immunization record through the CT WiZ Public Portal, a free service provided by the Connecticut Department of Public Health Immunization Program.


Patients and legal guardians can access official immunization records for themselves and their children using the CT WiZ Public Portal.

How to access a record on CT WiZ

  1. Go to CT WiZ Public Portal (you must be using Google Chrome, Microsoft Edge, or Mozilla Firefox)
  2. Enter your or your child's name, date of birth, and gender
  3. Select your relationship to the patient whose record you are accessing
  4. Enter the phone number or email address you or the patient used when registering for the vaccination
  5. Click "Search" to locate the record*
    *A search result will only appear if there is an exact match in CT WiZ of your name, date of birth, and cell phone or email address used at registration for the vaccine.
  6. Click "Request Code to Access Immunization Record" and enter the access code sent by text or email to confirm your identity
  7. Click "Download" to view, save, or print your Official Immunization Record

Troubleshooting & Frequently Asked Questions

I am receiving an error message that my record was unable to be found. What happened?

An exact match is required when you search. This includes your name, date of birth and email or cell phone used. Please note, parents and guardians cannot pull up their adult children’s records.

Troubleshooting tips:
  • Use the exact name you used during registration at the clinic. Doublecheck your vaccination appointment confirmation or related records to see if you may have used a nickname or alternate name.
  • Use the email or cell phone you used at the clinic registration.
  • Ensure you correctly typed the information when searching on the CT WiZ Public Portal.

You won’t find the record if the contact information does not match what the provider reported to CT WiZ. If you are unable to locate your record, you can contact the State Immunization Program for assistance (Phone: 860-509-7929 or Helpdesk: Helpdesk form).

My record is incomplete/missing vaccination doses. Why?

Your provider may not yet have reported the vaccination doses to CT WiZ. Contact your provider for assistance. If you cannot reach your provider, you can contact the State Immunization Program for assistance (Phone: 860-509-7929 or Helpdesk: Helpdesk form).

I received an error message saying that I have exceeded my log-in attempts and am now locked out. What happened?

If you exceed the search attempt limit, you will be temporarily locked out. You will be able to try again later.

Why am I getting a “Login” page?

Your internet browser may be showing a cached version of the website. Clear your browser's cache and click the Public Portal Link again.

2. What other ways can I obtain my/my child’s immunization record? 

  • Check with your doctor or public health clinic.
  • Check with your school.
  • Call the CT Department of Public Health Immunization Program at (860) 509-7929. With proper identification, you can receive a free copy of your/your child’s immunization record.

3. What are the benefits of CT WiZ for parents and children?
Research has shown that both parents and doctors overestimate the rates at which children are fully immunized. Parents are often unaware of immunization schedule recommendations and doctors often overlook 1-2 immunizations. This is becoming an increasing problem with the rapid changes in immunization recommendations for children.

CT WiZ can:

  • Give providers easy access to a permanent record of your child’s shots even if you move away from Connecticut or your baby’s doctor retires.
  • Let a doctor find your child’s history in a computerized database if you forget the record or have difficulty obtaining it.
  • Give parents the official immunization records needed for daycare, school or camp.

4. Is my child enrolled in CT WiZ?
If your child was born in the state of Connecticut since January 1, 1998, he/she was enrolled at time of birth through birth certificate information. Parents may elect not to have their child enrolled by sending a signed written request to opt out of CT WiZ. Please include your child's full name and date of birth. By opting out, your child's shot record will no longer be available in CT WiZ. DPH will not be able to provide you with a record of your child’s vaccinations.
