To obtain a license to operate hoisting equipment as defined above:

1. Complete the online application on the State of Connecticut eLicense website.
2. Provide a notarized statement from your employer indicating the dates and duties of employment operating such equipment or proof of ownership and control of a company utilizing such equipment.
3. Return all documents, including the appropriate fees to the Office of the State Fire Marshal.
