An organization that transports blind or disabled persons can request a disability placard online. You can use this service to apply for a disability placard, or renew / replace your placard. You can request a renewal of a renewable disability placard within 6 months of its expiration. Temporary placards cannot be renewed.

You will need:

  • Organization name
  • Business number issued by Connecticut Secretary of State
  • Vehicle plate number (must have active Connecticut registration status)
  • A valid email address or mobile phone number
  • Documentation showing your relationship to the organization (for example, letterhead or business card)
  • A credit card (for temporary placard transactions only)
  • Completed and signed placard application (Permanent Placard Application Form or Temporary Placard Application Form)
  • Your Social Security number (optional)
