DEEP Grant Application Requirements

  • Application procedures vary by grant program.  Application forms may be obtained from the appropriate agency contact.
  • In addition to specific grant program application forms, the following supplemental paperwork may be required for receipt of funds from DEEP:
    • Signature Resolution:
      • Indicates authority for the party to enter into agreement with DEEP
      • Indicates the legal name of the party entering into the agreement with DEEP
      • Indicates the title of the individual authorized to sign agreements with DEEP
      • May indicate the name of the individual authorized to sign agreements with DEEP
    • Incumbency Certification - Indicates the name of the incumbent official authorized in the resolution to enter into agreements with DEEP
    • Non-discrimination Certification required for all contracts with non-governmental entities, regardless of the value of the contract.  Complete the form appropriate for you or the entity you represent as applicable for this contract.
    • CHRO Forms - Provides information regarding affirmative action practices which is required by the Commission on Human Rights and Opportunities for any funding award in excess of $4,000.00
