Subject:          Field Training Documentation

As you are already aware, any probationary police officer graduating from a Basic Recruit Training Program that commences on or after July 1, 2001 will have to successfully complete four hundred (400) hours of field training before said officer can be certified.

I have enclosed two new forms that should be used to document an officer's field training requirement:

-Recruit Proficiency Check-Off List -Recruit Proficiency Check-Off List (Field Training & Evaluation Program -FTEP- 4) -This form will replace the old DFTR-2 "Departmental Training Checklist." After this form has been completed by the Field Training Officers, it should be retained in the probationary officer's personnel file at your agency.

-Certification of Field Training Requirement -Certification of Field Training Requirement (FTEP-4A) -This form replaces the old DFTR-l "Certification of Supervised Departmental and Field Training Requirement (40-40) form." After this form has been completed it should be sent to:
-Certification of Field Training Requirement (FTEP-4A) -This form replaces the old DFTR-l "Certification of Supervised Departmental and Field Training Requirement (40-40) form." After this form has been completed it should be sent to:

Please use the new forms for all probationary officers who graduate from any Basic Training Program that starts on or after July 1,2001.
