You will need to create an account and log into Business.ct.gov to submit a paper filing online. Fees for submitting a paper filing online can be paid securely using Mastercard, Visa, Discover, or Amex. 

For filings submitted by postal mail or in person, please make checks payable to: Secretary of the State.

No forms are available for conversions. Instead, you will need to manuscript a form according to the applicable Connecticut General Statute. This is a complicated transaction, and legal advice is highly recommended.
