What information should I provide to the Taxpayer Advocate?

Form CT-TPA, Request for Assistance from the Office of the Taxpayer Advocate
- We recommend that you DO NOT send us your Social Security number, federal employer identification number or Connecticut tax registration number via email.
- Your telephone number and the hours you can be reached,
- Your previous attempts to solve the problem, and the office you contacted,
- The type of tax return and year(s) involved, and
- Description of the problem with any substantiating documentation.
We are not responsible if your electronic mail is lost, intercepted or misused by others.
For security reasons, we will not provide any account information in response to email questions.
If you want to authorize another person to discuss the matter with the agency, download form LGL-001, Power of Attorney, and send it to DRS. However, DRS will continue to send all correspondence directly to you.
