What information should I provide to the Taxpayer Advocate?


NOTE:  The Taxpayer Advocate will be unable to assist you unless the form below is completed in its entirety and sent with all the required information.

  • We recommend that you DO NOT send us your Social Security number, federal employer identification number or Connecticut tax registration number via email.
  • We are not responsible if your electronic mail is lost, intercepted or misused by others.

    For security reasons, we will not provide any account information in response to email questions.

  • Your telephone number and the hours you can be reached,
  • Your previous attempts to solve the problem, and the office you contacted,
  • The type of tax return and year(s) involved, and
  • Description of the problem with any substantiating documentation.

If you want to authorize another person to discuss the matter with the agency, download form LGL-001, Power of Attorney, and send it to DRS. However, DRS will continue to send all correspondence directly to you.
