You cannot request a duplicate non-driver ID online if the below scenario is true for you:

Step 1: Gather what you’ll need

  • Your first and last name exactly as it appears on your current non-driver ID
  • Your date of birth
  • Your street address
  • Your non-driver ID number
  • Your social security number
  • A credit or debit card

If your non-driver ID is lost or destroyed, you can get a duplicate non-driver ID.

 

If your name has changed since you got your last non-driver ID, there are additional steps to update your name on your new non-driver ID. Learn how to change your name on your non-driver ID.

Step 1: Print and complete the required form

Step 2: Make an appointment

When requesting a duplicate non-driver ID, please make an appointment at a DMV office, AAA location, Milford and North Haven Nutmeg State Financial Credit Union or West Haven City Hall.

NOTE: AAA locations, Nutmeg State Financial Credit Union and West Haven City Hall cannot replace commercial driver’s licenses, commercial learner’s permits or licenses with passenger endorsements, and no Drive Only licenses. These locations may also charge a convenience fee up to $8.00 per DMV transaction.

Step 3: Get your non-driver ID in the mail


You will get a temporary non-driver ID at your appointment. Your permanent ID card will arrive through the mail.

 

Your new ID will only be mailed to the mailing address on file with the DMV. If you have mail forwarding, you will NOT receive your new ID.


Your duplicate credential will be mailed directly to the address on file and you will receive it within 20 days.

To request a duplicate non-driver ID by mail:

  • Complete a REQUEST FOR A CONNECTICUT DRIVER'S LICENSE/IDENTIFICATION CARD BY MAIL (form B-350).
  • Mail form and payment to:

    Department of Motor Vehicles
    60 State Street
    Wethersfield, CT 06109
    Attention: Central Issuance Operations Unit
  • Checks should be payable to the “DMV.”
  • Your duplicate credential will be mailed directly to the address on file and you will receive it within 20 days.
