Transitional Living Program

CMHC’s Transitional Living Program is an unlocked 10-bed unit which provides a step-down level of care. Length of stay is individualized, with a focus on accomplishing the tasks necessary to secure housing and to obtain the financial and emotional supports needed to facilitate successful transition into the community. Eligible individuals are encouraged to participate in structured day programs in the community, while recovery-oriented group programming is provided on the unit for those who are less able to tolerate structure. Individuals must be referred by CMHC clinicians, and although the majority of clients are admitted from an inpatient unit, some are admitted from Ambulatory Services to stabilize and avoid the need for hospitalization.
